Moving this here, but I tested it a bit more, and the error just seems to appear whenever I take damage from an enemy as any character. I tested T.Lost, Lost, Isaac, and Blue Baby, and the error appeared both when taking contact damage from enemies as well as from bullets. The error didn't appear when taking damage from curse rooms, fire obstacals, or when holy mantle prevented damage, but it did appear when I took damage from red poop.

It's possible it's caused by another mod interacting with this one, as I have many mods and it would just take a long time if I wanted to remove them all to test it without them, but the error is from this mod specifically. The error also appears on the Repentogon console, so you probably won't see it unless you install Repentogon and launch the game via that, since Repentogon displays errors much better than the vanilla client.
